These services will provide in-home care for those individuals with a mental illness and/or cognitive impairments through:
In addition, these specialized services will provide support to patients’ families through caregiver education and counseling. We also assist patients and their families in coordinating care with other available community resources. Staffed by experienced Psychiatric Nurses, Social Workers, CNA’s, Occupational and Physical Therapists, these comprehensive services will develop a plan of care as ordered by their physician. Continued physician consultation will be performed by the visiting nurse through scheduled care plan reviews and updates. Home Bound Healthcare is also very fortunate to have a Psychiatric Medical Director, Dr Pradeep Thapar, to assist when needed. Ellen Tierney RN, BSW, LNHA Director of Psychiatric Services Ellen Tierney has 32 years of healthcare and management experience. Her expertise includes extensive psychiatric and chemical dependency nursing, marketing, staff training within Long-Term Care facilities, Hospitals and Community Out-Reach programs. Maureen Prestegaard RN-C Admissions Coordinator Maureen Prestegaard has 25 years of nursing and management experience. Her expertise in behavioral health and chemical dependency extends from youth to older adults. During her nursing career in Long-Term-Care and Hospitals, Maureen successfully managed nursing departments, trained staff, and provided utilization review functions. Suzanne Blanchard LCSW, C-ASWCM Clinical Director / Geriatric Care Manager Suzanne Blanchard has 10 years of social worker experience in hospitals, Long-Term Care and non- profit organizations. Her career concentration has been working with the chronically mentally ill and older adults. As a recent member of the National Association of Geriatric Care Managers, she has continued to expand her services within case management, counseling and advocacy.
